Appointments & Referrals

To make an appointment for primary care or immunizations, please log onto the MHS GENESIS Patient Portal or call the Central Appointment Desk at 707-423-3000, Monday - Friday, 7 a.m. - 3:15 p.m.  

DGMC Referral Processes:

Step 1 - Referral received from PCM
Step 2 - Visit Referral Management at the Patient Service Center the same day you received your referral
Step 3 - The PSC will activate your referral or book your specialty appointment if applicable and update your contact info
 
Location: Second Floor (2C – use south entrance and walk towards the lab/radiology area)
No appointments needed at PSC
Hours: Monday - Friday, 7:30 a.m. - 4:30 p.m.
