Internal Medicine
The Internal Medicine Clinic at David Grant Medical Center provides primary care services for medically complex retirees and adult dependents of active-duty members. Internal Medicine is a specialty which focuses on the prevention, diagnosis, and treatment of adult medical conditions. With the treatment facility’s eldest (average age exceeds 80 years-old) and most complex patients, Internal Medicine specialists leverage a multidisciplinary team to maintain and enhance patient quality of life. The clinic offers both face-to-face and telephone appointments which are available on either an acute or routine basis.
We manage many chronic conditions. If any condition is beyond the scope of practice of clinicians in our clinic, patients will be referred to a
specialist within DGMC or to TRICARE network
Acute Appointments:
Acute appointments are for new non-life-threatening illnesses that require patients to be seen by a clinician within 24 hours. Please call 707-423-3000 to request a same-day appointment. We strive to maintain acute appointment availability to prevent the need for Emergency Room or Urgent Care visits. Beneficiaries, Beneficiaries, dependents/retirees may utilize TRICARE network
urgent care centers if acute appointments are unavailable.
For severe conditions, threatening loss of life, limb or eyesight, go to the nearest ER.
Routine Appointments:
Routine appointments are for chronic conditions or new minor illnesses or injuries which can wait to be addressed well beyond a 24-hour timeline.
Routine appointments include:
- Annual Wellness exam
- Management of Chronic Conditions such as Hypertension, Heart Disease, Diabetes, Asthma, COPD and Obesity
- Pap smears
- Osteopathic Manipulative Treatment (OMT)
- Occupational Exams
- Pre-Operative Clearances/Optimization
- Family and Medical Leave Act (FMLA) paperwork
- Nursing Home admission paperwork
- Handicapped Placard/DMV paperwork
- Physician Orders for Life Sustaining Treatment (POLST) forms
Phone and Virtual Appointments available for:
- Medication clarification
- Medication refills
- Lab and imaging results
- Referral renewals
Walk In Services:
Because of the advanced age and medical complexity of our patients, we do not operate Clinical Support Staff Protocols (CSSPs) but have nurses readily available to assist with triaging your needs. Please book a face-to-face appointment with your PCM for completion of the required paperwork.
Please bring your paperwork to your appointment with all patient and employer information completed.
If you are receiving specialty care, you will need to bring medical records to the appointment or provide records to IMC prior to your appointment, if records are not available in the GENESIS Patient Portal (PA to HYPERLINK to Genesis Patient Portal Login).
Prepare for Your Appointment:
When you see your primary care manager, please prepare by:
- Bringing a current medication list or a bag of all your medications
- Noting which medications need refills
- Bringing a blood sugar log if you are diabetic
- Bringing a blood pressure log if you have hypertension (high blood pressure)
- Making a prioritized list of any concerns or questions
- If a specific referral is needed, please bring the desired Physician’s name, address, phone and fax numbers
Please plan to arrive 15 minutes before your appointment time to allow for any gate delays and unpredictable travel conditions, especially during inclement weather. While out of our patients’ control, delays can put the patient at risk of losing their appointment slot and needing to be rescheduled. In addition, early arrival allows you the time to fill out any appointment paperwork. Please call
