The 60th Medical Group provides ready reliable care to 34,000 TRICARE Prime beneficiaries at Travis Air Force Base and hosts 1,498 mobility positions. What is Patient Centered Medical Home (PCMH)?
PCMH is an interdisciplinary team-based health care delivery model for patient care with you in the center and led by your primary care clinician. PCMH utilizes a team to provide comprehensive and continuous medical care to patients with a goal to obtain maximal quality care while achieving an excellent patient care experience. PCMH facilitates partnerships between patients, their clinicians, and when appropriate, the patient's family.

- Please stop by the Patient Service Center (PSC) once your provider has entered a referral and before you depart the medical facility. You may also call at 707-423-7641 to activate referral.
