Family Medicine Residency
The Family Medicine Residency Clinic provides routine, preventive, and acute outpatient care for enrolled DoD beneficiaries. We aim to improve the health of our beneficiary population by providing safe, quality, evidence-based health care.
We manage many chronic conditions. If any condition is beyond the scope of practice of clinicians in our clinic, patients will be referred to a
specialist within DGMC or to TRICARE network.
Acute Appointments:
Acute appointments are for non-life-threatening illnesses that require patients to be seen by a clinician within 24 hours. Please call (707) 423-3000 to request a same-day appointment, if available. Beneficiaries, dependents/retirees may utilize TRICARE network
urgent care centers if acute appointments are unavailable.
AFMAN 41-210/4.11.4.: Unit commanders and supervisors have the authority to grant up to 24 hours sick status at their discretion if a member’s illness/injury does not require military treatment facility (MTF) intervention.
For severe conditions, threatening loss of life, limb or eyesight, go to the nearest ER.
Routine Appointments:
Routine appointments are for illnesses not causing severe symptoms.
Routine appointments include:
- Annual Wellness exam with your clinician
- Management of Chronic Conditions such as Hypertension, Heart Disease, Diabetes, Asthma, COPD and Obesity
- Pap smears
- Occupational Exams
Phone and Virtual Appointments available for:
- Medication clarification
- Medication refills
- Lab results
- Referral renewals
Walk In Services: (ages 3-65 years old)
Walk-in hours are:
Monday to Friday, 8AM-3PM
Walk in Services include:
- Sore throat, cough and cold
- Urinary tract infection (UTI)
- Sexually Transmitted Infection (STI) screening (ages 18-65)
- Pregnancy test
- Wart treatment (provider must eval prior to first treatment)
- Injections (Testosterone/Depo-Provera/B-12, (with provider order)
Administrative Services:
School Physical paperwork
Paperwork can only be completed via drop off if your child has been seen for a school physical with in the past 60 days with no change in health. Otherwise, you will need to contact the appointment line to schedule an appointment.
Exceptional Family Member Program (EFMP)/Permanent Change of Station (PCS) Paperwork
You will need to make an appointment with your PCM and bring any pertinent specialty care documents and paperwork with you including EFMP PCS packet. Advise calling to schedule 3 month prior to PCS.
Family and Medical Leave Act (FMLA) & Disability Paperwork
If you have a current diagnosis and need FMLA or Disability paperwork completed for yourself or to care for a family member, you will need to
make an appointment with your PCM.
Please bring your paperwork to your appointment with all patient and employer information completed.
If you are receiving specialty care, you will need to bring medical records to the appointment or provide records to FHC prior to your appointment, if records are not available in the GENESIS Patient Portal.

- Minor Derm procedures
- Vasectomy
- Newborn circumcisions
- GYN/OB services
- Geriatrics clinic
- Cast/fracture clinic
- Behavioral Health
Prepare for Your Appointment:
When you see your primary care manager, please prepare by:
- Bringing a current medication list
- Letting your provider know what medication refills are needed
- Bringing a blood sugar log if you are diabetic
- Bringing a blood pressure log if you have hypertension (high blood pressure)
- Making a prioritized list of any concerns or questions
- Appointments are 20 min long and your provider may only be able to address 1-2 concerns at each appointment.
Please plan to arrive 15 minutes before your appointment time due to allow for any gate delays and unpredictable travel time, especially during inclement weather. While out of our patients’ control, delays can put the patient at risk of losing their appointment slot and needing to be rescheduled. In addition, early arrival allows you the time to fill out any appointment paperwork. Please call 707-423-3000 as soon as possible if you are unable to make your scheduled appointment to cancel or reschedule.
